- Schedule through the Vaccine Administration Management System (VAMS)
- (VAMS is a secure, web-based tool built to help jurisdictions, vaccination clinics, organizations/employers, and vaccine recipients manage COVID-19 vaccination efforts. It supports operations and data collection and tracking to meet COVID-19 vaccination requirements. It is being utilized by local and state health departments to schedule immunizations for eligible recipients.

- Schedule through your healthcare provider
- Healthcare systems such as UCONN and Hartford Healthcare host their own clinics. Please see their web pages for further instructions.
- Schedule through CVS
- CVS Pharmacies in Avon, Bristol, Coventry, Glastonbury, Guilford, Hamden, North Haven, and Uncasville stores will begin administering COVID vaccine on February 11th. Booking of appointments will begin as early as February 9th.
- Call the access line
- CT COVID Vaccine Appointment Assist Line at 877-918-2224 Mondays through Fridays from 8:00 a.m. to 4:30 p.m.
- Due to high volume, this option is only recommended for individuals who have not had success with the above option

In response to COVID-19 outbreak, effective immediately, FVHD asks that you do as much paperwork as possible through electronic means, i.e. phone 860-352-2333, email, (a list of staff email addresses can be found on our website www.fvhd.org), fax 860-352-2542 and USPS.
Credit card payments can be made over the phone, and we accept checks through the mail. All applications and FVHD files can be viewed and downloaded from www.fvhd.org. Take note that processing time for applications of any kind will be much longer than usual as all non-essential work is not being completed at this time. We greatly appreciate your cooperation to help us slow the spread of coronavirus (COVID-19).
